New Athlete Enrollment
KEEN currently has a waiting list children and youth with disabilities who wish to join our programs. To enroll your child in any KEEN program, it is important to get on the waiting list as soon as possible. We do add athletes to our program on a regular basis.
Enrollment Procedures
To enroll your child into a KEEN Program, please print and complete the following forms:

Liability & Publicity Waiver
To enroll your child into KEEN Swim, please print and complete the following form:
Swim Enrollment Form
To enroll your child into KEEN Bowling, please print and complete the following form:
Bowling Enrollment Form
Important Note: The Swim & Bowling forms will need to be returned with the Athlete Enrollment Form and the Liability & Publicity Waiver to have your child enrolled to participate.
